Request to be removed as DPS

This page allows you to inform the Council that you wish to be removed as the Designated Premises Supervisor (DPS) from a licensed premises. 

Complete either the Notice by an individual to cease being DPS form pdf , or Notice by an individual to cease being DPS form.doc and send the completed form to:
